Case Management at Alternatives

Since 1979, Alternatives has been assisting individuals in need. The Community Outreach Services Department offers Community Support Services (CSS) for adults with mental health and housing needs in Somerset County. CSS is managed by an interdisciplinary team of professionals whose credentials include Licensed Clinical Social Workers, Licensed Professional Counselors and Trauma Informed Professionals, along with staff with advanced degrees in the Human Services field. This department has more than 30 years of experience providing expert mental health services, housing, and linkages to community resources.  Alternatives is licensed, accredited and an approved Medicaid provider. 

Alternatives’ Community Support Services are funded, in part, by the Somerset County Department of Human Services.

Individuals eligible for our Case Management Services are:

  • Have a primary Mental Health diagnosis
  • Are at least 18 years of age
  • Be living in Somerset County

What Sets Us Apart

Wrap-around services are tailored to each individual client to serve their particular needs. Our person-centered philosophy is considerate of all cultures, experiences and ideals, and it provides all persons served the support they need to assist them in achieving their goals – in their own way.  Services are intentional and meaningful. They are geared toward helping each individual become successful in the community.

 Services include, but are not limited to:

  • Advocacy
  • Life skills education
  • Mental health education
  • Referrals and linkages to external resources

Alternatives offers a 24/7 on-call system, with several tiers of support, including Directors and Associate Vice-Presidents, ready to assist clients and staff.
